Referee Booking Procedure This procedure is still be revised. Once it is ready it will be posted.
Sponsor Intro Letter It contains a letter and a form for sponsorship. Please note it is a "word document" that has to be altered in a couple of places to fit your team.
Tournament Cheque Request Form A document to be filled out and submitted to the club house when entering tournaments.
Rep Uniform Order Form This form will be used for ordering this years uniforms. Remember to make 2 copies, 1 for yourself and the other goes to the clubhouse.
House League
House League Rules 0809 These are the House League rules for the 2008 / 209 Season. Please make sure all players read this document.
Non Residency Form House League This form is for HOUSE LEAGUE players who do not reside in Mississauga and which to play in our organization. There are no fees involved .
Referee Application This form is used for applying to be a referee. You must also register as a volunteer for one of the house league sessions. Fill the form in and mail it to the club house.
